The RN Supervisor is responsible for supervising onsite RN’s while they handle all aspects of health care for the individual in collaboration with the treatment team to ensure each individual achieves their maximum level of health and wellness by assessing, planning, implementing, coordinating, and evaluating options and services to meet an individual’s health care needs through communication and available resources to promote quality, cost-effective outcomes.

Primary Responsibilities:

  • Demonstrate knowledge of and implement Northeast Arc Policy and Procedures, including the Code of Conduct, Residential Division Standards and Funding Source Regulations. 
  • Ensure that the human, civil and legal rights of all individuals are protected.  Follow DPPC and mandated reporting guidelines. Teach and promote dignity, choices and reasonable risk with individuals.
  • Utilize a PBS approach when working with staff and individuals, including promoting the agency objectives related to personal growth, community involvement and decision making
  • Establish good interpersonal relationships with staff, helping them feel valued and appreciated.
  • Follow HIPAA guidelines and maintain confidentiality.
  • Maintain a safe work environment by following Universal Precautions
  • RN Case Manager is responsible to communicate and collaborate with Program Directors, to obtain and review medical records, current medications, health status and functional abilities.
  • Responsible to develop communication and collaborative relationships with all medical and Ancillary Services involved in the individual’s care.
  • Responsible to act as an advocate and liaison for the individual to assure that they are receiving a level of care consistent with clinical best practices.
  • Responsible to participate in the development and presentation of educational work shops and person-specific trainings.
  • Responsible to provide MAP training for eligible direct service professionals                                                              
​​​​​​​Case Management Responsibilities:
  • Conducting a comprehensive nursing  assessment of the individual’s health and psychosocial needs and develop a Case Management Plan collaboratively with the individual, family and caregivers
  • Develop a Health History for each individual admitted to the Residential Division
  • Responsible in conjunction with The Program Director, to Develop and maintain Health Care Plan for each individual supported in the Residential Division. Section 2a: Medical Plan and Section 2b  Nursing Care plan
  • Responsible to review, modify and update the Health History biannually, consistent with the ISP and Annual Physical
  • Complete DDS Health Care Record and Health and Dental within ISP timelines.
  • Medical Plan of Care and Nursing Care Plan are maintained and modified quarterly. 
  • Responsible to Develop individualized protocol and intervention to address specific medical conditions. Case manager is responsible to review, modify if necessary annually and if a change in status has occurred.
  • Collaborate with the interdisciplinary team to advocate for the member in achieving clinical and life goals.
  • Maintain documentation of health care encounters, case management communication and monthly key data in the Section three of the Health Care fact sheet
  • Episodic Case Management for hospital admissions and discharges.
  • Medication Reconciliation  whenever a transfer occurs 
  • Case manager,  in collaboration with Program Director,  will  review the Medical Book quarterly
  • Other duties and special projects as assigned.

  • Must be fully licensed as a Registered Nurse in Massachusetts and must maintain licensure from the Massachusetts Board of Registration in Nursing. Minimum three years nursing experience and three years’ experience in the direct care of the elderly or disabled adults is required.
  • Must have the eligibility to become a MAP Coordinator
  • Must maintain CPR certification and complete required programmatic trainings.      
  • Must have ability to coordinate/collaborate with community support agencies; experience and adaptability to work with families and interdisciplinary teams.
  • Must have proficiency with computers, including Microsoft Office, electronic health records, etc.
  • Must drive to various locations for home visiting purposes and medical appointments.
  • Light physical effort might be required.
