Description

Job Summary:  This Individual Support Provider plays an important role in the daily life of a young man with Cerebral Palsy. This young adult is highly engaging and is passionate about advocating for his peers with special needs. The supports provided are designed to enhance the individual’s ability to have meaningful engagement in all daily activities, which include but are not limited to, daily employment, community involvement and recreational outings. 

 

Major Responsibilities:

  • Participates in the planning of “Agency with Choice” activities with the consumer and the family. Notifies family of all activities and issues that may arise during activities.
  • Implements goals and objectives related to recreational and leisure time activities.
  • Assists individual with personal hygiene, ADL’s (activities of daily living, toileting, bathing and dressing) and mobility
  • Support coordination of transportation via THE RIDE, public transportation or drive individual using family conversion vehicle. 
  • Arranges, as needed, for medical/emergency treatment of the consumer and adheres to the Family Support procedure guidelines.
  • Completes all required documentation to ensure appropriate service delivery and compliance with DDS, and agency standards (Mileage, summary sheets, fact sheets and incident reports).
  • Attends all required trainings and meetings.
  • Other duties and special projects as assigned.

Qualifications:
  • Minimum 18 years of age and High School Diploma/equivalent required.
  • First Aid, CPR and PBS certifications, or willing to obtain.
  • Experience with individuals with developmental and physical disabilities.  
 

Physical Demands: 

  • Assist with Activities of Daily Living (dressing, undressing, bathing, toileting, etc.)
  • Provide support with Wheelchair transfers
  • Manage access of individual utilizing a wheelchair in/out of vehicle, areas of employment
  • Must be able to be mobile and active
  • Must be able to work on your feet for 30 min or more
  • Must be able to lift at least 50lbs multiple times throughout a shift.
