Description
Job Summary: To provide quality living supports and be responsive to individuals and Providers at all times. Ensure that the Northeast Arc Residential Division reflects the mission statement and vision of the agency.

Major Responsibilities:

  • Provide Case Management and oversight of individuals in Shared Living.
  • Demonstrate knowledge of and implement Northeast Arc Policy and Procedures, including Residential and Shared Living Standards and Funding Source Regulations.
  • Ensure that the human, civil and legal rights of all individuals are protected. Teach and promote dignity, rights of choices and reasonable risk with individuals.
  • Work as part of the Residential Team, including assisting peers as needed.
  • Utilize a PBS approach when working with staff and individuals, including promoting the agency objectives related to personal growth, community involvement and decision-making.
  • Effectively communicate within the Northeast Arc and any outside agencies, families, guardians, medical professionals, day programs, etc.
  • Act as a liaison with local DDS offices and other state agencies.
  • Assure quality in program implementation on a day-to-day basis.
  • Identify and address unmet needs of any Individual or Provider. Coordinate and provide supports as needed, including respite to Shared Living Providers.
  • Collaborate with Providers to support people in living full lives, including promoting individuals’ health and well-being through the provision of health care and opportunities for a healthy lifestyle.
  • Monitor and oversee the implementation of ISP goals and objectives. Attend individual ISP’s.
  • Conduct at least Monthly Home Visits and provide additional oversight that ensures Provider’s homes are clean and provide a safe living environment, and that individuals are receiving appropriate supports.
  • Maintain and follow a current safety plan, including training staff, conducting fire drills and reporting any issues.
  • Complete required documentation completely and accurately, including ISP Documentation, Health Care Records, HCSIS, Incident Reporting, Case Management notes and House Committee Reports.
  • Ensure systems, including home books, are in compliance with all Northeast Arc Policies, Residential and Shared Living Standards, QA Tools and DDS Survey and Certification.
  • Ensure self and staff have required trainings. Ensure Providers, Respite Providers and Shared Living Relief are fully trained and have the skills to most effectively support the individuals.
  • Be responsive to all out of hour calls from the Providers, Individuals and Guardians. Provide coverage for other Shared Living team members as needed.
  • Ensure the Residential Team are aware of daily schedule and staffing coverage. Keep them informed of any pertinent issues and changes in status, including providing clinical information to Weekend On-Call team.
  • Participate in, Clinical Meetings, team meetings, QAs and other meetings as deemed necessary.
  • Be responsive to phone calls and emails in a timely manner.
  • Demonstrate sound judgment and decision making, including being able to assess a situation, develop alternative solutions and decide the appropriate course of action required.
  • Adjust priorities, schedule and supports provided to respond to the critical needs of the program. Seek assistance from appropriate people when necessary.
  • Be approachable and responsive to Providers and staff.
  • Provide direct support to the individuals as needed.
  • Provide staff supervision, including performance appraisals and when necessary, disciplinary actions.
  • Establish good interpersonal relationships with staff, helping them feel valued and appreciated. Provide staff the opportunity for personal growth and career development.
  • Complete billing information, respite payments, other documentation accurately and in a timely fashion.
  • Have a working knowledge of computer systems, including Microsoft Office and Kronos.
  • Follow HIPAA guidelines and maintain confidentiality.
  • Participate in Northeast Arc committees and /or training groups as assigned.
  • Other duties and special projects as assigned.

Education and Experience:

  • BS/BA and 3 years Human Service Experience or MA and 2 years Human Service Experience.

Skills, Knowledge and Abilities:

  • A desire to assist people in achieving their dreams and goals.
  • Maintain certifications as required in Northeast Arc Policy and Procedures, including but not exclusive to: Vehicle Safety, Human Rights and Fire Safety.
  • Maintain a valid Massachusetts driver’s license.
  • Apply a high work standard; establish criteria to achieve a high level of service delivery

Physical Demands:

Must be able to work at a desk/computer in an office environment and be physically able to provide direct support to individuals in their homes.

 

Work Environment:

This position includes a combination of working in an office environment and also working on-site in community settings, including Individuals and Providers homes.
