Job Summary:  This Individual Support Provider plays an important role in the daily life of a young man with Cerebral Palsy. This young adult is highly engaging and is passionate about advocating for his peers with special needs. The supports provided are designed to enhance the individual’s ability to have meaningful engagement in all daily activities, which include but are not limited to, daily employment, community involvement and recreational outings.  
 

Major Responsibilities:

  • Works with family, individual and Agency with Choice Facilitator to ensure individual meaningfully and productively engages in work, community and recreational settings.
  • Participates in the planning of independent living activities with the individual and family. Notifies family of all activities.
  • Assists individual with personal hygiene, ADLs (activities of daily living: toileting, bathing and dressing) and mobility. 
  • Support coordination of transportation via THE RIDE, public transportation or drive individual using family conversion vehicle.
  • Plans and implements session dates, times and duration according to parameters of DDS allocation, and to the extent feasible, at the convenience of the individual.
  • Completes all required documentation to ensure appropriate documentation of support needs and service delivery in compliance with DDS and agency standards (To include ISP documentation, summary sheets, facts sheets and incident reports).
  • Reports hours worked in a timely way, and provides prompt written notification and explanation of cancellation of any scheduled sessions to family and Northeast Arc.
  • Attends all required trainings and meetings, including supervision and ISP meetings.
  • Other duties and special projects as assigned.

Qualifications: 
  • Minimum 18 years of age and High School Diploma/equivalent required.  
  • First Aid, CPR and PBS Certifications, or willing to obtain.
  • Experience with individuals with developmental and physical disabilities. 

Physical Demands: 

  • Maintain a safe work environment as determined by Northeast Arc,
  • Follow Universal Precautions at all times,
  • Wear protective equipment (i.e. gloves, masks, back belts, eye wear, etc.) as necessary,
  • Assist with Activities of Daily Living (dressing, undressing, bathing, toileting, etc.)
  • Provide support with Wheelchair transfers
  • Manage access of individual utilizing a wheelchair in/out of vehicle, areas of employment
